We look forward to seeing you for your pre-cataract evaluation.

Please make sure you bring:

• Your driver’s license / legal identification
• Insurance card
• A list of all medications you are currently taking
• The bottle for any eye drops you are currently using

If you wear contact lenses, they will need to be removed prior to your evaluation. Please remove contact lenses and begin wearing glasses for:

• Soft lenses: 5 days
• Toric lenses: 14 days
• Hard lenses: 3 weeks

If you are a new patient to our office, you may find it helpful to complete our intake paperwork and bring it with you to your appointment.

All patients are required to wear a mask while in our office to protect our patients and staff. Unless necessary, please plan to arrive at our office without a guest.
